Role of Cytokeratin 5/6 in Differentiation between Benign and Malignant Breast Tumors among Sudanese females patients

Abstract

Abstract This is a hospital based analytical retrospective case control study conducted at military hospital (Omdurman) and Radiation and Isotope Center (RICK) (Khartoum), and Alneelain University - college of medical laboratory science, during the period from February to June 2017. The study aimed to investigate the role of CK 5/6 in differentiation between benign and malignant breast tumors, using immunohistochemistry. Fifty nine paraffin blocks were collected from patients previously diagnosed as breast tumor, 30 (50.8%) of them were malignant diagnosis samples and 29 (49.2%) of them were benign diagnosis samples. The paraffin blocks were cut by rotary microtome, and then stained by immunohistochemistry method (Avidin biotin technique). The data obtained were analyzed using SPSS program. Frequencies mean and Chi-square test values were calculated. The age of patients ranged between 18 to 80 years with mean age of 40 years. The study revealed that most patients were 40 and more than 40 years representing 32 (54.2%) and the remaining 27(45.8%) were less than 40 years. The histopathological diagnosis of patients revealed invasive ductal carcinoma in 22(37.3%) samples, metaplastic carcinoma in 3(5.1%), medullary carcinoma in 3 (5.1%) sample, fibroadenoma in 25(42.4%) samples, fibrocytic changes in 4(6.8%) samples. The grade of tumors (invasive ductal carcinoma) showed grade I in 1 (3.3%) sample, grade II in 6(20.0%) samples, grade III in 15 (50%) samples, and not graded (other than invasive dutal carcinoma) in 8 (26.7%) samples. Result: Positive expression of CK 5/6 was found in (5/30) in malignant samples and (25/30) samples showed negative expression, while all benign samples (29/29) showed positive expression of CK 5/6, this result showed significant association of CK 5/6 expression and benign breast tumors (P. value = 0.000). Conclusion: The study concluded that positive expression of CK 5/6 was associated with benign tumors of breast. vi الخلاصه هذه الدراسة عبارة عن دراسة تحليلية استطلاعية أجريت في مستشفى السلاح الطبي بامدرمان والمركزالقومي للعلاج بالأشعة والطب النووي )الخرطوم(، وجامعة النيلين كلية علوم المختبرات - الطبية، خلال الفترة من فبراير إلى يونيو 2017 . الدراسة تهدف إلى التحقيق في السايتوكيراتين 5/6 في التفريق بين الأوارم الحميدة والخبيثة للثدي بإستخدام كيمياءالأنسجة المناعية . تم جمع تسعة وخمسين قالب شمعي من عينات مرضى كانو مشخصين مسبق ا على أنهم مصابين بأورام الثدي وكان 30 ( 50.8% ( منهم عينات خبيثة و 29 ( 49.2% ( منهم كانت عينات حميدة. قُطعت القوالب بإستخدام المشراح الدوار وصُبغت بواسطة طريقة كيمياءالأنسجة المناعية ) طريقةالبايوتين أفيدين ( واستخدم برنامج الحزم الإحصائية للعلوم الإجتماعية لتحليل البيانات حسب متوسط التكرار وقيم إختبارمربع كاي. تراوحت أعمار المرضى بين 18 إلى 80 سنة مع متوسط عمر 40 سنة. وكشفت الدراسة أن معظم المرضى كانوا 40 عاما وأكثر من 40 عاما يمثلون 32 ( 54.2% ( والباقي 27 ( 45.8% ( أقل من 40 عاما. كشف التشخيص للمرضى عن سرطان الأقنية الغازية في 22 عينة ) 37.3% (، سرطان ميتابلاستيك في 3 ( 5.1% (، سرطان النخاع في 3 عينة ) 5.1% ( ، ورم ليفي في 25 عينه ) 42.4% ( ، تغيرات فيبروسيستك في 4 عينات ) 6.8% .) وأظهرت درجة الأورام )سرطان الأقنية الغازية( النوع الأول I في عينة ) 3.3% (، النوع الثاني II في 6 من العينات ) 20.0% ( ، والنوع الثالث III في 15 من العينات ) 50% ( ، وليس متدرجا )عدا سرطان الاقنيه الغازية( في 8 ( 26.7% ( عينات. النتيجه: اظهرت الدارسة أن السايتوكيراتين 5 / 6 موجب الظهورفي ) 5/30 ( في عينات خبيثة و وسالب الظهورفي ) 25/30 (, في حين أظهرت كل العينات الحميده ) (29/29 موجبه الظهور. الملخص: أظهرت هذه النتيجة علاقة ذات دلالة احصائية ) قيمة P = 0.000 ( بين السايتوكيراتين 5/6 وأورام الثدي الحميدة. وخلصت الدراسة إلى أن ايجابيه ظهور عن السايتوكيراتين 5/6 كان مرتبطا بأورام الثدي
