Expression ofsurvivin in malignant Breast Cancer AmongSudanese female patientsin Omdurman Teaching Hospital

Abstract

Abstract: Introduction:Worldwide breast cancer, the most frequently occurring cancer in women, is a major public health problem, breast cancer is the most commonly diagnosed cancer sites in women in Khartoum-sudan,in 2009 the first national population –based cancer registry (NCR) was established in sudan .during 2009-2010,6771 new cancer cases were registered.of those 3646(53.8%) were in women and 3125(46.2%) were in men. Method: This is hospital based analytical retrospective study conducted atOmdurman teaching hospital in Khartoum and AlNeelain University, college of medical laboratory science, during the period from June2018 to october 2018.The study aimed to investigate the existence expression ofsurvivin marker in breast cancer, using immunohistochemistry technique. Fourty (40) paraffin blocks were collected from patients previously diagnosed with breast cancerand 40(100%) diagnosis malignant samples. The paraffin block were cut by rotary microtome, and then stained by immunohistochemistrytechnique (indirect streptavidin –biotin_ HRP). The data obtained were analyzed using SPSS program version 21(Frequencies, mean and Chi –square test values) were calculated. Result: The age of patients ranged between (21 – 80) years. The study revealed that most patients in 21-40years (20)and from 41-60years representing 13 and the remaining 7 from 61_80 years. The diagnosis of study samples include invasiveductal carcinoma in 33 (82.5%) samples, ductal carcinoma in situ in 2(5%) sample, invasive lobular carcinoma in 3 (7.5%) malignant phyllodes tumor 1 (2.5%) sample,mucinous carcinoma 1(2.5%). The grade of malignant samplesranged from grade I in 3 (7.5%), grade II in 9(22.5%) samples, grade III in 12 (30%) samples ,Whilesurviving negative expression grade I in3(7.5%)samples,grade II4(10.0%) samples, grade III5(12.5%)samples,Survivin positiveexpression was detected in (28/40) in malignant samples, and (12/40) samples showed negative expression .Survivin expression is associated with malignant form of breast tumor, and predominantly found in grade III.

المستخلص: الانتشار العالمي لسرطان الثدي اكثر ظهورا في المراة وهي اكبر مشكله صحيه بشكل عام ,في الخرطوم –السودان سرطان الثدي شائع تشخيصه.في عام 2009 تم تاسيس اول منشاة لتسجيل مرضى سرطان الثدي في السودان. هذه الدراسة عبارة عن دراسه تحليليه اجريت في مستشفي ام درمان التعليمي وجامعة النيلين-كلية علومالمختبرات الطبيه ’خلال الفترة من يونيو 2018الي اكتوبر2018 . الدراسه تهدف الي التحقيق من تشخيص سرطان الثدي عن طريق السيرفيفين ماركر. تم جمع اربعين قالب شمعي من عينات مرضي مشخصين مسبقا علي انهم مصابين باورام الثدي وكانت جميع العينات خبيثة .قطعت المقاطع بسمك 3 مايكروميتر من كل قالب شمعي باستخدام المشراح الدواروصبغت بواسطة الانسجة الكيميائية المناعية واستخدام برنامج الحزم الاحصائية للعلوم الاجتماعية النسخه 21 لتحليل البيانات. ترواحت اعمار المرضي بين21الي 80 سنه وكشفت الدراسه ان معظم المرضي كانوا علي النحو التالي21_40عام(20)ومن 41_60عاما يمثلون (13) والباقي (7) من 61_80 عاما . كشف التشخيص للمرضي سرطان القنوات الاجتياحي 33عينة (82.5%)’سرطانالفصيصي الاجتياحي في3 عينات (7.5%) ,السرطان الداخلي للقنوات 2عينات (5%) , ورم الفايلدس الخبيث في عينه واحده (2.5%) , السرطان المخاطي في عينه واحدة (2.5%). واظهرت درجة الاوارم الخبيثه موجبة في النوع الاولI في 3 عينات (7.5)’النوع الثاني II في 9 عينات(%22.5)’والنوع الثالث III في 12عينه (%30), والسالبه النوع الاول Iفي3 عينات(%7,5)’النوع الثاني II في4عينات(%10)’النوع الثالث IIIفي 5 عينات(12,5%). اظهرت الدراسة ان سيرفيفين موجب الظهور في(28/40) وسالب الظهور في (40/12) في العينات الخبيثة. السيرفيفين يرتبط مع اورام الثدي الخبيثىة واكثر وضوحا في الطور الثالث.
