Nursing Documentation Process Among Nurses at Ibrahim Malik and Khartoum Bahri Teaching Hospitals at Khartoum State (2018)

Abstract

Abstract Introduction: Nursing documentation offers written evidence of patient evolution, it has to contain foundations and the primary critical thinking behind clinical decisions, interventions, and evaluations of caregivers consequently it must fulfill with recognized standards and quality Objective: This study conducted to assess nursing documentation process at Ibrahim Malik and khartoum bahri teaching hospital at Khartoum state. Methods: descriptive cross sectional hospital based study , data collected used questionnaires and adopted check lists during period of January to February 2019, data analyzed using statistical package for social science computer program (version 20 ) ,then using descriptive state inform of frequency ,percentage . The data collected according multi stage, stage one hospital selection, hospital selected purposely, each public hospital from each town to generalized the result for Khartoum state , stage tow participant selection by chosen the first inter of ward and whom available in time of data collection. Tow hindered thirty seven questionnaire was fielded by participant, and nineteen document available in ward used to filled adopted check list Result: Most of studied nurses were female 75.5% about half of study sample characterized by good level of qualification, (28.7%) have diploma degree in nursing, and , (49.8%) who represented have Bachelor degree revealed good knowledge of them, Also the result guarantee that the study sample characterized by good level of work experience 51.5%. The study reflected 66.4% of nursing knowledge which opposition with their practice 10.5%. Obstacle facing documentation process were (overload of work, shortage of staff, time pressure and facilities) were (75.9%, 53.2%, 39.7 and 32.1%) respectively and accessibility of documentation facilities (protocol, written or electronic system) was44.5%. Also there were positive association between qualification and their knowledge towards nursing documentation process (p-value: 0.033) and positive Association between nurses’ qualification and their practice towards nursing documentation process. (P-value: 0.006). But unluckily the study show negative Association between nurses’ experience and their practice of nursing documentation process. . (P-value: 0.086) Conclusion: It is concluded the nurse have good knowledge of nursing documentation and poor application of it. Recommendations: The main recommendations initiate clear guide line and set off written or electronic system to improve nursing documentation process.

المقدمة: وثائق التمريض تقدم ادله مكتوبه من تطور المريض ويجب ان تحتوي علي معلومات المريض الاساسيه والتفكير السليم لاتخاذ القرارات السريريه ,والتدخل التمريضي وتقييم مقدمي الرعايه ويجب ان تفي بالمعايير والجوده المعترف بها . الهدف :تهدف هذه الدراسه لتقييم عمليه التوثيق التمريضي في مستشفي ابراهيم مالك ومستشفي الخرطوم بحري ب ولايه الخرطوم . المنهجيه : دراسه وصفيه حيث تم جمع البيانات باستخدام استبانات وقوائم تحقق في الفتره من يناير الي فبراير 2019 حيث تم تحليل البيانات بواسطة برنامج التحليل الاحصائي والعلوم الاجتماعيه(اصدار 20) واستخدام البيانات المحلله علي هيئه تكرار ونسب مئويه . تم جمع المعلومات علي مراحل :المرحله الاولي مرحله اختيار المستشفيات حيث تم اختيار مستشفي حكومي عام لكل مدينه وذلك بغرض امكانيه تعميم التائج علي ولايه الخرطوم ,المرحله الثانيه مرحله اختيار المشاركين حيث تم الاختيار حسب الدخول لكل قسم في زمن جمع العينات حيث تم جمع عدد 237 استبانه و19 قوائم تحقق من الملفات المتوفره النتائج : اظهرت الدراسه ان معظم الممرضين المدروسين من الاناث 75.5في المائه و حوالي نصف عينه الدراسه تميز بمستوي تعليمي جيد .28.7 في المائه يحملون شهاده دبلوم في التمريض , 49.8 في المائه يحملون شهاده البكالريوس .كما تميزت عينه الدارسه بمستوي جيد للخبره 51.5 في المائه . توصلت الدراسة الي ان مستوي معرفة الممرضين بالتوثيق التمريضي جيدة (60.4%)الذي يتعارض مع مستوي تطبيقهم (10.5%) كما اظهرت الدراسه ان المعوقات التي تواجه عمليه التوثيق هي ( ضغط العمل,قله الاصطاف,الزمن ,عدم توفر الامكانيات بنسب (23.1/,39.7/,53.2/75.9) علي التوالي .وتوفر معينات التوثيق ( بروتوكول ,نظام مكتوب او الكتروني ) بنسبه44.5%. هنالك علاقه ايجابيه بين المؤهل الاكاديمي ومعرفه الممرضيين بالتوثيق التمريضي (p-value: 0.033) وايضا هنالك علاقه ايجابيه بين المؤهل الاكاديمي والتطبيق العملي للتوثيق التمريضي) 006.p-value 0) ,للاسف العلاقه سلبيه بين سنين خبره المدروسين والتطبيق العملي للتوثيق التمريضي (p-value 0.086). الخلاصه :وخلصت الدراسه ان مستوي معرفه الممرضيين عن التوثيق التمريضي جيده مقارنه بالتطبيق. التوصيات :التوصيه الاساسيه الشروع في انشاء قواعد ارشاديه وانشاء نظام كتابي او الكتروني .
