Knowledge and Practice of Nurses Regarding Discharge Plan of Patients following Kidney Transplantation in Khartoum state -2017

Abstract

ABSTRACT Introduction; the advancement in transplant era provides the patient release from dialysis, reintegration of living in society and good quality of life. Transplantation is a lifelong therapeutic resource that enhances survival with great cost-effectiveness compared with other treatments. Methodology: This was descriptive hospital based study was conducted, aimed to assess Knowledge and Practice of nurses regarding Discharge Plan of Patient following Kidney Transplantation in Khartoum state -2017. Data was collected by a designed questionnaire and observational check list. Data was analyzed using Statistical Package for Social Sciences (SPSS) version16. Sampling: Total coverage of nurses working in renal units (ICU), recovery room, and outpatient follow up clinic and transplanted wards in, Ahmed Gasim and ibn sina hospital.In Ahmed Gasim (40) nurses and Ibn Sina hospital (20) nurse. Results: study revealed that 66.7%, and 73.3% of the study sample knew the indications, and requirements needed for transplant respectively. All respondents have the information about rejection as an important post-operative complication, 93.3% knew its different types, 73.3% of participants have knowledge regarding definition of immunosuppressive, and 86.7% knew its types. 100% stated that anti-rejection medications have side effects. 66.7% of the study population mentioned using mask as the main precautions used to protect from potential source of infection postoperatively. Results of practice showed that only 46% of respondent nurses have a poor practice regarding education of transplanted patients before discharge, majority of them deficit was time of education 26.7% of the participant nurses start education to transplanted patient immediately before discharge and it is incomplete. Conclusion: Participant's knowledge regarding discharge plan for transplanted patients seems to be good, but their practice regarding education of transplanted patients was poor and deficient. Recommendations: Organizing periodic educational sessions for transplant nurses is highly needed to increase their knowledge and improve their practice. Availability of educational leaflets to be provided to transplanted patients on discharging. Guideline and continuous training is mandatory. مستخلص الدراسة

المقدمة: إن التقدم المطرد لنتائج زراعة الكلى يوفر للمريض فرصة للتخلص من غسيل الكلي ، وإعادة إدماج المعيشة في المجتمع لأنه هو مورد علاجي دائم يعزز البقاء على قيد الحياة كما أنه أقل حيث التكلفة مقارنة مع العلاجاتالأخرى المنهجية: أجريت هذه الدراسة الوصفية في المستشفى بهدف تقييم المعرفة والممارسة للممرضين بشأن خطتهم لزارعي الكلى بعد الخروج من المستشفى في ولاية الخرطوم -2017. تم جمع البيانات من هذه الدراسة باستخدام استبيان صمم للدراسة بالإضافة لقائمة الملاحظة والرصد. تم تحليل البيانات باستخدام برنامج الحزم الإحصائية للعلوم الاجتماعية(SPSS) النتائج: أظهرت النتائج أن 66.7٪ و 73.3٪ من أفراد عينة الدراسة أجابوا بإجابات صحيحة بخصوص المؤشرات متطلبات الزراعه. كان جميع المستطلعين يعرفون رفض الجسم للعضو المزروع كأحد المضاعفات المهمة بعد العملية، 93.3٪ يعرفون أنواعه المختلفة، 73.3٪ من المشاركين كانت إجاباتهم صحيحة فيما يتعلق بتعريف المثبط للمناعة، 86.7٪ يعرفون أنواعه%100 ذكروا بأن الأدوية المضادة للرفض لها آثار جانبية 66.7٪ .من عينة الدراسة أكدوا بأن باستخدام الكمامة من الاحتياطات الرئيسية للحماية من أي مصدر محتمل للعدوى بعد العملية. أظهرت الدراسة أن 46% من الممرضين المشاركين ف البحث انهم ذو ممارسة جيدة عند تقديم الارشادات لمريض زارع الكلى عند خروجه والاعلبية مقدرتهم ضعيفه عند تقديم الارشاد 27% الخلاصة: معرفة المشاركين بشأن متطلبات البرنامج التعليمي للمرضى الزارعين تبدو جيدة، وممارساتهم فيما يتعلق بتعليم المرضى الزارعين كانت ضعيفة. التوصيات: أوصت الدراسة بتنظيم دورات تثقيفية دورية لطاقم التمريض بأقسام زراعة الكلى لزيادة مقدراتهم‘ تحسين ادائهم وتوافر المنشورات التعليمية لتقديمها للمرضى الزارعين عند خروجهم من المستشفى
