Nurse’s Competences regarding Care of Child on Mechanical Ventilator in Pediatric Hospitals Khartoum State 2017

Abstract

Abstract Background : Mechanical ventilation is a life-sustaining therapy for the treatment of pediatric patients with acute respiratory failure. It is a very common modality in intensive care units, and indeed the advent of its use heralded the dawn of modern intensive care units. The overall aim of study was to assess nurse’s competences regarding care of child on mechanical ventilator Methodology: a descriptive cross-sectional study design was used. Jaafer Ibn auf and Ahmmed Gasim hospitals ,forty five nurses working in PICU enrolled in the study , data was collected by using self-administered structured questionnaire and observational checklist ,ethical consideration was obtained .data was analyzed by using Statistical package for Social Sciences (SPSS) version 16 . Results: study reveal that less than half (48.8% )of nurses have good knowledge about types of mechanical ventilation, whereas half of nurses (51.1%) have good knowledge about mechanical ventilation modes suitable for children , Majority (86.7% )of nurses have poor knowledge regarding indications of mechanical ventilation. The majority (71.1%, )of nurses have fair knowledge regard criteria for extubation , About (60% )of nurses have v.good knowledge of measurements used to prevent infection in ICU . Less than half (46.7%) of the nurses knowledge about the complications of mechanical ventilation was good. Eighty four percentage of nurses who work in intensive care unit wash their hands before entering intensive care unit , and most of them (71.1%) not Wear mask. Sixty two percentage of nurses they monitor child need for suction, major of them (88.8%) hyper oxygenate the child before procedure. All of nurses they Give ordered medication as prescribed (100%). The majority (91.1% ) Document vital signs and ventilator parameter in sheet. And their Documentation for each procedure was done about (68.8%) . Conclusions: the researcher was concluded that the level of nurses knowledge was fair , in spite of their level of practice was good.

مستخلص البحث الخلفية : تعتبر التهوية الميكانيكية هي العلاج المنقذ لحياة مريض الفشل الرئوى الحاد.وهو علاج شائع الاستخدام فى وحدات العناية المكثفة وقد بشر ادخاله فى الحقل الطبي بفجر جديد من وحدات العناية المكثفة الحديثة .تهدف الدراسة لتقييم كفاءة الممرضات والممرضين وممارستهم فيما يتعلق برعاية الطفل على جهازالتنفس الصناعي . منهجية البحث : تم استخدام تصميم دراسة وصفية مقطعية شملت عينة الدراسة عددخمسة واربعون ممرض وممرضة يعملون بغرفة العناية المكثفة بمستشفيات جعفر بن عوف ومستشفى احمد قاسم للاطفال . تم جمع البيانات عن طريق الاستبيان الذاتى وقوائم فحص الملاحظات . تم مراعاة الدواعى الاخلاقية عند جمع البيانات , تم تحليل البيانات باستخدام حزمة التحليل الاحصائى للعلوم الاجتماعية النسخة السادسة عشر. النتائج :اظهرت الدراسة ان48.8 % من عينة البحث كانت لديهم معرفة جيدة حول انواع التهوية الميكانيكية . ونصف العدد 51.5% لديهم معرفة جيدة حول انماط التهوية الصناعية . ومعظم عينة البحث بنسبة 86% كانت معرفتهم ضعيفة حول دواعي التهوية الميكانيكية كما وجدت الدراسة ان اغلب الممرضين 71.1% معرفتهم متعادلة حول ايقاف التهوية الميكانيكية ومعظمهم 60% كانت معرفتهم جيدة جدا حول الطرق المستخدمة لمنع العدوى فى غرف العناية , اقل من النصف 46.7 % لديهم معرفة اقل حول مضاعفات جهاز التهوية الميانيكية . ومن الجانب العملى وجدت الدراسة ان معظم الممرضين 84.% يغسلون ايديهم قبل دخول غرف العناية واغلبيتهم 71% يرتدون كمامات الوجه الواقية , 62% من عينة البحث يقومون بمراقبة حوجة الطفل الى شفط السوائل ومعظمهم 88.8% يمدون الطفل بالاكسجين قبل عملية الشفط . كدلك اثبتت الدراسة ان كل الممرضين يعطون الادوية كما موضح لهم والاغلبية منهم 91% يقومون بتسجيل العلامات الحيوية وعلامات جهاز التنفس الصناعى ومعظمهم 65% يقومون ايضا بتسجيل كل الاجرات التى تقدم للمريض . الخاتمة : استنتج الباحث ان مستوى المعرفة لدى الممرضين كان متعادلا والمستوى العملى لديهم كان جيدا .
