Knowledge of the Nurses Regarding Discharge Plan of Child with Chronic Disease In Gaffer Ibn ouof Referring Hospital 2018

Abstract

Abstract Discharge plan is processes that aim to improve the coordination of services after discharge form hospital by considering the patient need in the community. Chronic conditions in childhood are defined as those having a biological, psychological or cognitive basis, which have lasted or have the potential to last for at least one year. This study aim to assess knowledge of the nurse regarding discharge plan of child with chronic disease. This ibn Ouof Referring hospital .The sample of the study was 96 nurses who were available and agree to involve in the study, during the study period. Data has research of descriptive analytical nature; the researcher uses a questionnaire to collect the data. The populations of the studies are nurses whom work in gaafer been preceded statistically by use a questionnaire and analyzed by SPSS program version (22). The research reach in to Result which show that: the majority (71.9%) have bachelor degree about 62.5% have poor level of knowledge regarding meaning of chronic diseases, most of the study sample 60 (62.5%) did not know that the discharge plan start on admission. There is significant association between experience and meaning of discharge plan with p.value( 0,000) also there is significant association between experience and level of knowledge of nurse regarding important of discharge plan with P value(0,001),there is significant association between qualification of nurse and knowledge about health care provider involve in discharge plan with P value (0,000) and there is significant association between qualification of nurse and level of knowledge of the nurse about persons who involved in discharge plan with p value(0,000). The researcher recommends that continuous education and in servers training programs is very important for nurses.

ملخص البحث تعرف خطة الخروج من المستشفي هي العملية التي تهدف الي تحسين تنسيق الخدمات الصحية بعد خروج المريض من المستشفي مع مراعاة احتياجات المريض في المجتمع وتعرف الحالات المزمنة في الطفولة بانها تلك التي لها اساس بيولوجي او نفسي او ادراكي؛ والتي استمرت او لديها القدرة علي الاستمرار لمدة عام علي الاقل ؛ تهدف هذه الدراسة الوصفية الى تقيم معرفة الممرضات وممارستها فيما يتعلق بخطة خروج الاطفال الذين يعانون من امراض مزمنة في مستشفي جعفر بن عوف المرجعي وكانت عينة الدراسة 96 ممرض وممرضة كانت متاحة ووافقو علي المشاركه خلال فترة الدراسة تم جمع البيانات عن طريق الاستبيان وتحليلها بواسطة برنامج(الحزم الإحصائية للعلوم الاجتماعية) واظهرت الدراسة ان غالبية 69(71.9%) لديهم شهادة البكالوريوس (62.5%) من الممرضات لديهم مستوي (ضعيف) من المعرفة فيما يتعلق بمعني الامراض المزمنة؛ ومعظم عينة الدراسة 60 (62.5 %)لم يكن يعلم ان خطة الخروج تبدا عند دخول الطفل الي المستشفي يعكس هذا المستوي الضعيف من المعرفة حول الوقت اللازم لبدء خطة الخروج هناك ارتباط كبير بين الخبرة العملية و معرفة معني خطه الخروج(000,0) pوايضا هناك ارتباط كبير بين الخبرة العملية ومستوي معرفة اهمية خطة الخروج(0,001)pكما ان هناك ارتباط مهم بين التجربة العلمية ومعرفة مشاركة مقدم الرعاية الصحية المشارك في خطة الخروج(000,0)p وهناك ارتباط كبير بين تاهيل الممرضين ومستوي معرفة الاشخاص المشاركين في خطة الخروج(000,0)p . وقد توصل البحث الذي تم التوصل الية النتائج ؛ الي ان العديد من الممرضات لديهم معرفة ضعيفة بخطة خروج الاطفال المصابين بامراض مزمنة لذا توصي الدراسة بان برنامج التعليم والتدريب المستمر مهم جدا للمرضات.
