Abstract Background: The risk of thromboembolic complications is greatest during the first three months after surgery for both mechanical and bioprosthetic devices, with persistent life-long risk for patients with mechanical valves. Furthermore, the presence of atrial fibrillation, a common anhythinia in patients with VHD, also requires lifelong anticoagulation in the majority of patients. These considerations underscore the importance of addressing proper anticoagulation techniques to minimize postoperative thrombotic complications, while maintaining acceptable levels of risk related to bleeding“) . Anticoagulation regimens vary according to stugeon, nature of the valve mechanical or biological. The American College of Chest Physicians have made the following recommendations to protect patients with prosthetic heart valves from developing a stroke, For mechanical heart valves by Anticoagulation with Warfarin at an INR range 2-3 for patients with a mechanical valve in the aortic position, in the mitral position, an INR of 2.5-3.5 is recommended (2) Objective: To study the patients K.nowledge, attitude regarding warfarin therapy for patients with mechanical valve replacement. Research Methodology 1 Descriptive cross sectional hospital based study design conducted in Al- Sudan Heart-Center its contains (intensive care unit —- coronary care unit —cath lab - ward — emergency department- outpatient clinic) it provided medical and surgical services, population with different (age, six ,level of education , socioeconomic status , environment ,ethnicity and religions.)sample size total coverage for all patients came to outpatient during study period, data collection tool structured (questionnaire) technique by interview, list of study variable presented by quations asked to the patient and the patient answered ,questionnaire contain socio-demographic data ,knowledge of the patients ,attitude, regarding warfarin therapy, The data analysis By used — statistical package for social sciences — (spss)Version 15 Result: majoraty of the patients were male than female, majority of the patients take warfarin at the evening ,all the patients they believe the important of warfarin , most of the patients (78,6)% have good knowledge about the therapy less patients (21,4)% they don’t have knowledge about the warfarin therapy, The study conclude that majority of the patients they haven’t complication related to wari'arin therapy because they have good knowledge about warfarin therapy, All of the patients had positive attitude regarding warfarin therapy, in relation knowledge increased with age, study recommended to facilitate Check of international normalized ratio in primary health care, improve educational, applications program regarding warfarin therapy to increase health promotion.en_US
