Knowledge, Attitude and Practice of Health Professionals towards Infection Control Measures at Omdurman Teaching Hospital -Khartoum State 2017

Abstract

Abstract Introduction: Infection prevention and control measures aim to ensure the protection of those who might be vulnerable to acquiring an infection , both in the general community and in a healthcare setting. Healthcare related infection affects 1 in 10 patients admitted to the hospital . The aim of this study was to assess the knowledge attitude and practice of healthcare professionals, in Omdurman teaching hospital towards infection control measures. Methodology: Hospital-based cross-sectional study, conducted in Omdurman teaching hospital in 2017. Total coverage was used as a sampling technique for selection of the sample size. A pretested standardized self-administered questionnaire was used to collect data, the questionnaire was composed of four section background data, knowledge, attitude and practice . Standardized two checklists were used adopted from the WHO practical guidelines on infection control. Data were analyzed using SPSS version 24. Results: Total coverage was used(200) with a response rate of 80%. 161 health professionals employed in Omdurman teaching hospital. Doctors constituted (Doctors 118 (73), nurses 43 (26%)) .No specialist participated in the study, 72.% had good knowledge regarding infection control while 7% had poor knowledge. 68% had good practice level while only 4% had a poor level of practice cross-tabulation of knowledge and practice showed a statistically significant association (p-value =0.0). 73% had a good attitude and less than 2% had a poor attitude towards infection control measures. Attitude and practice had a statistically significant association (P-value of association using chi-square test=0.0). 79% of the participants did not receive any in-service training in infection control while only 20% received in-service training in infection control , of them only 3% had a post-implementation assessment and only 2% received incentives for adhering to the taught protocols. The hospital had no written policies or protocols . Facilities for hand washing were not readily available. 33% of the participants had undergraduate training in infection control. while 43% had never received in-service training in infection control. Conclusion: Healthcare professionals in Omdurman teaching hospital had a good level of knowledge, practice and attitude towards infection control. Although the hospital lacks clear infection control policies and protocols. As well as be deficient in of facilities for hand washing. Two thirds of the participants did not receive any undergraduate training in infection control . The majority of them had no in-service training in infection control .The hospital did not provide any incentives of any kind for those who showed adherence to the infection control guidelines . Recommendation: Encouragement of both undergraduate and in-service training in infection control. Development of infection control policies to promote more adherence to infection control guidelines .Provision of incentives and conduct of more researches in infection control.

مستخلص البحث المقدمة:معايير مكافحة العدوى هي معايير تهدف لحماية الاشخاص المعرضون لاكتساب العدوى في المجتمع وفي المرافق الصحية . العدوى المكتسبة في المستشفيات تصيب 1 من كل 10 مرضى يتم ادخالهم الى المستشفى , يهدف هذا البحث الى تقييم مستوى المعرفة والممارسة والسلوك للعاملين بالحقل الصحي بمستشفى امدرمان التعليمي منهجية البحث: أجريت دراسة مقطعية في مستشفى امدرمان للاطباء والممرضين باستخدام استبانة مهيكلة ومعايرة قياسيا وباللغة العربية تتكون من اربعة اجزاء هي البيانات الشخصية للمشاركين والمعرفة والسلوك والممارسة لجمع البيانات , التغطية الشاملة استخدمت لاختيار العينة الاحصائية , عدد 200 كادر صحي بمستشفى امدرمان التعليمي كما تم ايضا استخدام قائمة تدقيق واحدة للعنابر والاخرى لغرف العمليات اعتمدت من منظمة الصحة العالمية وتم تحليل البيانات باستخدام برنامج الحزمة الاجتماعية الاحصائية نسخة 24. النتائج: التغطية الشاملة للكوادر الصحية بمستشفى امدرمان (200) معدل الاستجابه كان فقط 80% شارك في البحث 161 طبيب وممرض(43 ممرض, و118طبيب) , 72% من المشاركين اظهرو مستوى جيد جدا من المعرفة وفقط 7% اظهرو مستوى ضعيف . 68% من المشاركين في البحث كانت مستويات ممارستهم لموجهات مكافحة العدوى جيدة جدا بينما فقط 4% كان مستوى التزامهم ضعيف هذا التقارب بين الممارسة والمعرفة كان له ارتباط زو دلالة احصائية (p value =0.000). 73% من المشاركين كان لديهم سلوك ايجابي نحو موجهات مكافحة العدوى بينما فقط 1% كان لديهم سلوك سلبي التقارب بين السلوك والمعرفة كان له ارتباط زو دلالة احصائية (P-value of association using chi-square test=.000). مستشفى امدرمان لي لديها سياسات او بروتكولات لمكافحة العدوى ولاتوجد مرافق لغسل الايدي متوفرة بصورة يسهل الحصول عليها . اغلب المشاركين لم يحصلوا على تدريب في مكافحة العدوى اثناء الدراسة الجامعية , اغلبهم لم يحصل على تدريب اثناء العمل كما ان الذين اظهرو التزام بموجهات مكافحة العدوى لم يتم تحفيزهم . الخاتمة: خلصت الدراسة الى ان مستوى المعرفة والسلوك والممارسة جيد جدا عند الاطباء والممرضين بمستشفى امدرمان التعليمي.المشفى ليس لديه سياسات او بروتكولات لمكافحة العدوى , اغلب المشاركين لم يحصلوا على تدريب اثناء الدراسة الجامعية او اثناء فترة العمل , لايتم تحفيز العاملين بشكل جيد. التوصيات: يوصى بإدخال التدريب المهني المستمر للعاملين في الخقل الصحي في مجال مكافحة العدوى وادخاله في المناهج الجامعية , وتبني الساسيات الصحية المناسبه للتقليل من العدوى المكتسبة في المستشفيات. وتحفيز العاملين في الخقل الصحي , وتشجيع المزيد من البحوث في مجال مكافحة العدوى.
