Competency of Nurses Midwives Regarding Care of Mothers during Fourth Stage of Labor at Omdurman Maternity Hospital, Khartoum, Sudan (September 2018 - March 2019).

Abstract

Abstract Background: Nurse Midwives are in a key position to disseminate knowledge and provide proper intensive nursing care during fourth stage of labour resulting in decreasing maternal mortality and morbidity. Objective: This study aimed to assess competency of Nurses Midwives regarding Care of Mothers during Fourth Stage of labour in Omdurman Maternity Hospital, (September 2018 – March 2019). Methods: This is a descriptive cross section hospital based took place in Omdurman maternity hospital, during the period of September – March 2019 to assess the competency of (72) midwives using total coverage sampling. Data was collected by using structured closed- end questionnaire to assess respondents’ knowledge, attitude In addition, a checklist used to follow respondents’ practice. The collected data statistically analysed by using (SPSS) version 21 and represented in tables of frequencies and percentages. Chi-square tests carried out to test significance and associations. Results: The study revealed that; the nurses had fair level of knowledge by (33.9% ), attitude and behaviour towards care of mothers during fourth stage of labour by ( 50.9% ), but they had poor level of practice about documentation by (64.1% ).In addition, it found nurses’ experience was significantly associated with knowledge, practice; moreover, the nurses’ qualification was significantly associated to knowledge, attitude and practice. Conclusion: Nurse Midwives in the current study showed poor level of knowledge in general, but the study editing updating knowledge of midwives regarding management of fourth stage of labour. Recommendations: The most important recommendation was health authorities should enhance update in-service training in order to improve knowledge, practice, and documentation.

خلفية: القابلات الممرضات يمثلن مفتاح رئيسي لنشر المعرفة والوعي وتوفير الرعاية التمريضية المناسبة خلال المرحلة الأولى من المخاض مما يؤدي إلى انخفاض معدل الوفيات والأمراض النفسية. الهدف: تهدف هذه الدراسة إلى تقييم كفاءة القابلات الممرضات فيما يتعلق برعاية الأمهات خلال المرحلة الرابعة من المخاض في مستشفى أمدرمان للولادة خلال الفترة (سبتمبر2018-مارس 201) المنهجية: هذه دراسة مقطعية مستندة إلى المستشفى استخدمت فيها طريقه التغطية الشاملة. تم جمع البيانات باستخدام الاستبيان المغلق لتقييم معرفة المجيبين وموقفهم وسلوكهم والتزامهم بالتوثيق؛ بالإضافة لاستخدام قائمة مرجعية لمتابعة الممارسة العملية للمبحوثات. وتمّ تحليل البيانات إحصائياً باستخدام برنامج (SPSS) الإصدار 21 لجدولة التكرارات والنسب المئوية لاستجابات الممرضات وكذلك أجريت اختبارات مربع كأي للارتباط. النتائج: كشفت الدراسة أن القابلات تمتاز بمستوى مقبول من المعرفة، ومستوى مقبول أيضاً من المواقف والسلوك تجاه رعاية الأمهات في المرحلة الرابعة من المخاض، كما وجدت الدراسة أن هناك مستوى ضعيف على صعيد الممارسة العملية والتوثيق. بالإضافة إلى ذلك، وجد أن خبرة الممرضات كانت مرتبطة بشكل كبير بالمعرفة والممارسة. وتوصلت الدراسة إلى أن هناك ارتباط معنوي ايجابي بين المؤهل العلمي ومستوى المعرفة والمواقف والممارسة لدى الممرضات. الخلاصة:خلصت الدارسة أن مستوى معرفة القابلات في الدارسة كان ضعيف بصوره عامة كما خلصت إلى أن التعليم أثناء الخدمة يساعد على تجديد المعرفة تجاه المرحلة الرابعة من المخاض. التوصيات: من أهم التوصيات ضرورة اهتمام السلطات الصحية بتحديث التدريب على رأس العمل من أجل رفع المعرفة وتحسين مستوى الممارسة العملية والتوثيق.
