    Expression of E-Cadherin and H.pylori in Sudanese patients with gastric adenocarcinoma and gastrointestinal stromal tumors.

Abstract. Introduction: E-cadherin (epithelial-cadherin), is a trans-membrane glycoprotein and is down regulated in gastric cancer. H. pylori (Helicobacter pylori) infection is associated with down-regulation of E-cadherin at the early stage of gastric cancer development. Testing E-cadherin and H.pylori expression can provide potential clinical applications for diagnosis, prognosis, and therapeutic targets in gastric cancer. The role of Ecadherins and H.pylori in the progression of gastric cancer and gastrointestinal stromal carcinoma has never been studied in Sudanese samples before. This study aimed to assess the effect of H. pylori infection and E-cadherin expression and their coexpression in gastric mucosa. Materials and Methods: Fifty consecutive paraffin tissue blocks from patients diagnosed with gastric cancer (GCs) and gastrointestinal stromal carcinomas GISTs were enrolled to assess E-cadherin and H.pylori expression using immunohistochemistry biomarkers. The patients median age was 60 and mean age was 55.08. P-value was calculated using Chi- squire test (P value <0.05 is significant). Results: There was 33 (66%) samples from males and 17 (34%) from females. Thirty five (71%) were diagnosed with adenocarcinoma of gastrointestinal tract (GCs) and 15 (29%) were Gastro Intestinal Stromal Tumor (GISTs). Out of the 50 samples; 4 (8%) were diagnosed with well differential adenocarcinoma, 18 (36%) diagnosed with moderately differential adenocarcinoma, 13 (27%) diagnosed with poorly differentiated adenocarcinoma and 15 (29%) were diagnosed with gastrointestinal stromal tumor (GISTs). Expression of H.pylori were detected in 8 (16%) of GCs while in 42 (84%) of the samples H.pylori protein were not expressed. E-cadherin, was expressed in 18 (36%) while in 32 (64%) E-cadherin was unexpressed. Histological differentiation of GCs and GISTs showed significant statistical association (P=0.02) with E-cadherin under-expression. H.pylori has significant statistical association with histological differentiation of GCs (P=0.01) especially moderately differentiated and poorly differentiated adenocarcinoma, however, H.pylori expression was not detected in GISTs and well differentiated adenocarcinoma cases. Moreover H.pylori and E-cadherin coexpression showed insignificant association (P= 0.391) with neither GCs, nor with GISTs. Discussion: Progression of gastric adenocarcinomas and GISTs is mainly dependant on under-expression of E-cadherin. H.pylori might play a role in GCs development, but it does not have a role in GISTs development. Our findings showed that E-cadherin can be used as biomarker for detection of GCs and GISTs in Sudanese patients.

    The roles of DOG1, CD117 and Ki-67 as immunohistochemisty biomarker in Sudanese patients’ diagnosis with GISTs

    Introduction: Gastrointestinal stromal tumors (GISTs) are potentially aggressive mesenchymal neoplasm. The use of Immunohistochemistry biomarkers for the differential diagnosis of GISTs is rarely used and no standard operating procedure has yet been established in Sudan. In this study we investigated the expression of three biomarkers CD117, Ki67 and DOG1 in GISTs in order to select the most accurate potential biomarker in the diagnosis of GISTs. Materials and Method: A total of 40 histopathological sections diagnosed with GISTs were recruited from Radiation and Isotope Centre of Khartoum (RICK)- histopathology laboratory. The study included 23 (57.5%) male and 17 (24.5) females. Age ranged from 11 to 90 years old. The three biomarkers CD117, Ki67 and DOG1 expression were tested in all the sections. Results: The mean age of the patients was 55.44± 18.43 (SD) years. Thirty six (90%) were positive for Ki67 and DOG 1 and 33 samples (85%) were positive for CD117. Four (10%) samples were negative for Ki67 and DOG1 and 7 samples (17.5%) were negative for CD117 marker. A highly significant association was noticed between Ki67 & DOG1 (P value 0.004). A higher association was between Ki67 and CD117 (P value 0.000). However there wasn’t a significant association between the expression of DOG1 and CD117 (P. Value 0.075). Discussion: This study showed that DOG1 and Ki67 are the biomarkers of choice in diagnosis of GISTs. For the first time in Sudan we showed that Ki67 is a biomarker that correlated with GISTs. It is already known that Ki67 remains in itsuse as a prognostic biomarker. Thus further studies can test Ki67 as a prognostic indicator for GISTs. We conclude that Ki67 and CD117 can be used as a biomarker in the diagnosis of GISTs.

    Sudanese slated fish (Kejeik) diet and risk to nasopharyngeal carcinoma- A case control study

    Abstract Introduction: Although nasopharyngeal carcinoma (NPC) is rare in most populations, it is a leading fomi of cancer in a few well-defined populations, including natives of southern China, Southeast Asia, the Arctic, and the Middle East/North Africa. The distinctive racial/ethnic and geographic distribution of NPC worldwide suggests that both environmental factors and genetic traits contribute to its development. Salted fish diet is common in population where NPC is more common. These foods can be very high in nitrates and nitrites, which react with protein to form nitrosamines which are chemicals that can cause damage to DNA. Salted dried fish known as Kejeik is commonly prepared in Sudan. Some studies has tested its nutfional factors but none has studied its relation to cancer specially NPC. In this study we aimed to investigate Sudanese salted fish known as Kejeik and its possible potential association with NPC in Sudan - Oial 4;.» uax .>\.1,>:|v;t;.. - uax .>t.\,,\J| as - .~,_¢_a\ .1»? Materials and methnds:One hundred and forty -two Sudanese’s participants (71 cases diagnosed with NPC, 71 nonnal healthy controls.), were enrolled in this study. Results: Age of cases was varying between (12— 75 years i 5), median (34, 35), mean age (38.44), and the age of controls was varying between (11- 75 years :1: 5), median (29, 30), mean age (36.72). The age distribution of cases showed a high peak at age 16-20 and another two equal peaks at age 26-30 and 31-35, and a third peak at age 56-60 with an observed decline in age groups of 66-75. There was 51 males and 20 females, with a ratio of about 5:2. A strong associated between Kejeik and NPC was P value 0.035 and OR 95%CI 0.44(0.20 to 0.95). Discussion: The monotonically increase in age group was scrutinized indicating a possibility of early life exposure to carcinogens. The widely held young age of onset, point to the likelihood of hereditary. Male to female ratio was 5:2 which is slightly higher than that known about NPC worldwide which is 3:2. Similar to Chinese salted fish, the Sudanese dried salted fish Kejeik is associated with NPC in Sudan. Carbaryl (1-naphthyl methylcarbamate), also known as Sevin, might have probably increased the carcinogenicity of Kejeik. Cconclusions: Results "obtained from the "present study -indicate that consumption of Sudanese salted fish known as Kejeik is a risk factor for NPC. Moreover, Sudanese NPC patients have the youngest age group peak in the world mainly among men. Key words: NPC, slated fish, Kejeik, Sudanese salted fish
